The Department of Health and Social Services (DHSS), Division of Senior & Disabilities Services, is offering Provider Agreements to qualified providers for the purpose of reimbursing travel expenses for providing Care Coordination services to eligible residents living in Rural Assisted Living Homes.
In order to establish a Provider Agreement, an applicant must submit proof of eligibility and a signed Provider Agreement to the following Person;
