This addendum addresses pre-bid meeting on Friday, February 27, 2015 @ 2pm at Crime Lab on 4805 Dr. Martin Lurther King Jr.
The date and time of bid closing is not changed : Wednesday, March 4, 2015 at 1:00 p.m.
Contract Intent: Removal of medical biohazard waste from the Department of Public Safety Scientific Crime Laboratory (Crime Lab), located at 4805 Dr. Martin Lurther King Jr. Ave, Anchorage, AK 99507. Establish a two (2) year contract with a two (2) year renewal options (Not-To-Exceed (NTE) $100,000) to pick up and destroy by incineration, or any other approved acceptable method (ie: Sharps, blood products, dirty gloves, etc). Price must include all containers, lids, and bags for collection and storage of waste material. Please see attached Scope of Services two (2) pages for more detailed information. Services are to be performed weekly and must include pick up at the DPS Crime Lab. Quantities shown are annual estimate. The State does not guarantee any maximum or minimum quantities.
Bids will be accepted until 1:00 p.m. Alaska Standard Time, Wednesday, March 4, 2015. Bids received after this date/time are considered to be late and will not be accepted.
